Referring your patients

Patients needing well woman, screening exams and obstetric care should be directed to their OB/GYN provider before seeing us.

If your patient experiences the following symptoms, you may send them directly to us:

  • Strong family history of ovarian cancer
  • Pelvic mass
  • Ovarian cysts 5 cm or greater in size
  • Vulvar lesions
  • Vaginal lesions
  • Abnormal pap smears with carcinoma in situ or high grade dysplasia
  • Fibroids and any symptoms of:
    • Abnormal bleeding
    • Pelvic pressure
    • Pelvic pain

Scheduling an appointment

You may call our office at (615) 340-4640 extension 0 or 224 to schedule a new patient consultation. We will need the patient's full name, address, phone number, their employer information, social security number and date of birth. You will also need to provide us with their insurance information and diagnosis.

Once we have scheduled the appointment, we request you fax ((615) 340-4642) us all medical information, including recent office notes, copies of ultra sounds, CTs, Pet scans, x-rays, lab work, and biopsies, etc. along with a copy of their insurance cards. We strive to get each patient in as quickly as possible and provide them with exceptional patient care.

Please ask your patient to complete the following pages and bring them to our office at their appointment time along with their insurance cards and driver's license.
